The following are general health and screening requirements as stated in the State of Rhode Island’s Rules and Regulations for School Health Programs.

General health examination requirements:

All 7th-grade students are required to provide proof of a general health examination and health clearance from a physician.  If your child has had a physical with their pediatrician, please provide the school nurse with a copy.  If there is no evidence that a general health examination has been performed, the school will make provisions for the district physician to provide a general health examination prior to the end of the school year.

General dental health screening requirements:

All 8th-grade students are required to provide proof of a dental screening/examination.  A general dental examination will be provided by the district dentist after the holiday break (date to be determined).  Students who are screened by a private dentist and who provide written documentation of the screening shall be exempt from the school dental screening.  Please contact the school nurse for yellow school dental card to be filled out by your child’s dentist in order to be exempt.

General scoliosis screening requirements:

Annual scoliosis screenings are required for all students in grades 6 through 8.  Parents may choose to have said scoliosis screening conducted by a private physician.  Please provide the school nurse with documentation and results of the screening.  Scoliosis screenings will take place in the spring (date to be determined).

Please feel free to contact the school nurse with any questions or concerns at 435-7819.
